Spells that fully occupy two adjacent breaches gain the bonus effects (if any) of both breaches.
Example: Sage's Brand (Cast: Deal 6 damage) is prepped to Breach III and IV. It will deal 8 damage when cast (6 base damage, 1 damage from Breach III, 1 damage from Breach IV).
